The role and importance of antimicrobials in the treatment of periodontal disease is a topic which is a source of ongoing debate. This Comment, published as part of the EBD Antimicrobials in Dentistry collection which is currently open to submissions, describes the outcome of a four-arm double blinded randomised controlled trial. It indicates that while the addition of metronidazole to mechanical debridement might add a very small marginal benefit to periodontal treatment regimes, the gains acquired are small and do not bring about clinically significant positive changes. Equally, full-mouth disinfection offers little advantage over traditional scaling and root planing.

As the world becomes increasingly aware of the dangers of over-prevalent use of antimicrobials, it is important that the dental world weighs very carefully the costs versus the benefits of every prescription that the profession writes.
